In addition to the enrollment packet, for any child who is not enrolled in public school ABC ACADEMY will need a copy of the child's immunization record and a health statement from their doctor stating the child is clear to participate in a child care program. Once the completed forms are returned you will receive a copy of the Parent Handbook which includes a form that will require a signature.
